*Case Manager (LPN or RN)

The Case Manager is responsible to assist in the development, planning, coordination and administration of the activities of Utilization Review and Discharge Planning. Including but not limited to daily review of medical records to determine appropriateness and medical necessity of admission, continued hospital stay and use of ancillary services
Education: Graduation from an accredited Nursing School
Experience:
Three years experience in case management or utilization management preferred. RN or LPN with at least one year experience in medical/surgical with excellent communication skills and ability to work collaboratively with all members of the health care team.
Licensure/Certification: Current NM Nursing Licensure; Current BLS Certification or obtain per policy upon hire; maintain such certification
